A Silver Alert has been issued for the Phoenix Police Department. This is the Arizona Department of Public Safety. The Phoenix Police Department is looking for Charles "Chuck" Mader. Chuck was last seen in the area of 34th Place and Flower Street. Chuck suffers from a medical condition that may cause him to appear confused or disoriented. Chuck is 5 feet 9 inches tall, 190 pounds, with grey hair and blue eyes. Chuck was last seen wearing a grey t-shirt with Angry Scotsman Brewing written on it, black shorts, and black Asics brand running shoes.
